My next steps

Your next steps

  1. Write down the plan you discussed

Use the worksheet to record medicines, monitoring, follow-up, and the questions you want to ask. Your care team supplies the individual instructions.

  2. Ask about everyday care

Ask how food, activity, medicines, and monitoring fit your own plan, including what to do when you are unwell.

  3. Prepare for follow-up

Bring your questions and any records requested by your care team. Let them know about costs, supplies, or instructions you find difficult.

Questions you can ask

  • Which first step matters most?
  • Can my medicines cause low glucose?
  • Can you show me how to use my device?
  • What should I do when I am sick?
  • Which checkups are due?

Let your care team know if cost, transportation, language, supplies, or unclear instructions make a step difficult.
