My next steps

Your next steps

For a medical emergency, call 911 in the U.S. or 112 in India; elsewhere, use your local emergency number.

  1. Write down what you want to discuss

Use the paper sheet to describe a change or question in your own words, along with when you noticed it.

  2. Ask how to share the information

Ask your care team when and how they want you to report changes. Follow their contact instructions.

  3. Bring your notes to the conversation

Keep the notes with your current medicine list and visit instructions. This record helps you explain what you have noticed.

Questions you can ask

  • What should I keep track of?
  • When and how should I report a change?
  • What details would help you understand my concern?

Let your care team know if cost, transportation, language, supplies, or unclear instructions make a step difficult.
